Each service user within your home must have risk assessments within their care plan.

What is risk assessment?
A risk assessment seeks to answer four simple, related questions:
Is there a need for action?
How bad?
How often?
What can go wrong?
It is not usually possible to eliminate all risks but care staff have a duty to protect service users as far as ‘reasonably practicable’. This means you must avoid any unnecessary risk.

In risk assessment we look at:
• hazards – which are situations with the potential to cause harm; and
• risks – which are defined as the probability that a specific adverse event will occur in a specific time period or as a result of a specific situation.

– Risk is the combination of likelihood and consequence of a hazard being realised.

 Decide whether it is significant and whether it is appropriate and sufficient controls or contingencies are in place to ensure that the risk is properly controlled
.
